Abstract:

A passive fixation, body implantable lead assembly has a cylindrical tip electrode whose outer side surface is covered with a thin dielectric insulating layer so as to mask an active, disk-shaped electrode surface at the distal extremity of the tip electrode. The active electrode surface preferably has an area less than about 1 mm. An MCRD, in the form of a collar, may be carried by a proximal portion of the tip electrode. Supplementing a set of main tines projecting from the distal end portion of an insulating sheath housing the lead assembly is a set of nubby auxiliary tines extending from the sheath immediately adjacent the distal extremity thereof.

Abstract:

An omnidirectionally steerable obturator facilitates the delivery of the distal tip of an introducer sheath into the coronary sinus of a heart. The steerable obturator comprises an obturator body extending longitudinally along a central axis, the obturator body being configured to be received by the introducer sheath. The obturator body further has a flexible, deflectable distal end section terminating in a rounded distal tip. An actuator, controllable from a proximal end of the obturator body, is operatively associated with the flexible distal end section of the obturator body to cause deflection of the flexible distal end section of the obturator body in at least one selected direction to facilitate passage of the distal end section of the obturator body and the distal tip of the introducer sheath into the coronary sinus of the heart. The obturator body is preferably configured to be received in a close fit within at least the tip of the introducer sheath. More preferably, the distal end of the obturator body has an outer surface and the fit between the outer surface of the obturator and at least the tip of the introducer sheath comprises substantially a line-to-line fit.

An implantable lead having an elongated lead body that includes an electrical conductor extending between a distal end of the lead body and a proximal end of the lead body is disclosed. The lead further includes an electrode formed at the distal end of the lead body, the electrode being coupled to the electrical conductor and one or more passive fixation mesh sections coupled to the distal end of the lead body that promotes tissue over growth.

Abstract:

An implantable stimulation lead system comprises a lead including a lead body dimensioned for placement inside the coronary sinus region. The lead system further comprises a device dimensioned for insertion within a lumen in the lead, the device including a main body; a steering knob secured to a proximal extremity of the main body; and a flexible distal portion secured to a distal extremity of the main body. The main body has a length such that, with the main body of the device substantially completely advanced within the lead, the flexible distal portion of the device projects distally from the aperture in the distal tip of the lead body. The flexible distal portion of the device may comprise a proximal section and a distal section, the distal section being more flexible, and thus softer, than the proximal section. In accordance with another aspect of the present invention, there is provided a device for delivering a body implantable lead, an embodiment of which device is as described above. The proximal and distal sections of the flexible distal portion of the device may comprise a unitary structure, with the distal section comprising a thin, very flexible leaf. Further, the flexible distal portion of the device may include a wire coil surrounding the proximal and distal sections of the flexible distal portion. In accordance with yet another aspect of the invention, there is provided a method of implanting an electrode of an endocardial lead at an implantation site within a cardiac vein accessible via the superior vena cava (SVC), coronary os and the coronary sinus region, utilizing a lead system as described above.
