Abstract:
An instrument for removal of an object, e. g. , a blood clot lodged in a blood vessel, has a longitudinally extending support that defines an axis. The support has a flexible distal section and a rigid proximal section. The flexible distal section has a compressed state, in which it defines a first path relative to the axis. A core-wire extending along the axis has a relaxed state in which it defines a second path relative to the axis. An anchor disposed on the flexible distal section and attached to the core-wire causes the flexible distal section to follow the same path. An actuator engaged to a proximal end of the core-wire enables a surgeon to selectively apply a tensile force thereto. This tensile force causes the core-wire and the flexible distal section to transition together between the first path and the second path.

A composite guidewire comprising a proximal wire segment having a proximal end, a distal end and a longitudinal axis extending therebetween, the distal end being formed with a first flat planar bonding face which extends parallel to the longitudinal axis, the proximal wire segment having a first elasticity; a distal wire segment having a proximal end, a distal end and a longitudinal axis extending therebetween, the proximal end being formed with a second flat planar bonding face which extends parallel to the longitudinal axis, the distal wire segment having a second elasticity; wherein the first and second flat planar bonding faces of the proximal wire segment are formed complementary to one another; and further wherein the proximal and distal wire segments are joined together by bonding the first flat planar bonding face to the second flat planar bonding face in an overlapping manner and a method for forming a composite guidewire.

Abstract:
A temperature-controlled actuator has a housing () with a proximal end and a moveable distal portion. A core-wire () extends along the housing, with its distal section anchored to the distal portion of the housing. The core-wire's distal section has an austenite state and a martensite state. The distal section moves the distal portion by transitioning between the austenite and martensite states in response to a temperature change along a thermometric section () of the core-wire. A proximal section in mechanical communication with the core-wire's distal section transmits tension, provided by a tensioning element (), to the distal section. The tensioning element, which is coupled to the proximal section of the core-wire, constantly applies a tensioning force to the core-wire.

The outer catheter may be positioned within a patient's heart and a portion of the outer catheter may provide movement like that of a windshield wiper by manipulation of a spring. By combining the windshield wiper type movement with the extension and rotation of the inner catheter relative to the outer catheter, a large area within a patient's heart may be readily mapped for conduction tissue and be subjected to laser ablation if appropriate. Various configurations provide flexibility in relatively positioning of windows used to apply laser energy and ring electrodes used to sense conduction tissue.
