Abstract:

A system and method for providing medication management via use of a network, such as the Internet, is disclosed. Within the system, a patient is diagnosed to determine a patient illness and then prescribed medication to treat the diagnosed patient illness. A medication compliance level of the patient is then determined and the patient's medication usage is managed via the Internet in accordance with the determined medication compliance level of the patient.

Abstract:

A system and method for scoring and managing patient progression is disclosed. A transceiver provides communication capabilities between a patient, a care provider, and/or a physician. A memory is communicably coupled to the transceiver, having logic therein that defines functions to be performed by the system. A processor is configured by the memory to perform the functions of: prescribing a care plan for the patient based upon medical conditions of the patient; performing an initial medical assessment of the patient; performing a medication compliance assessment of the patient; modifying the care plan based upon the initial assessment and the medication compliance assessment; and modifying the care plan in accordance with new assessments performed in association with medical progress of the patient. Answers to questions asked during the initial medical assessment of the patient, and during the new assessments, are scored for comparison purposes, thereby providing an accurate mechanism for monitoring patient progression.

A system and method for providing a patient care plan to assist in improving health of a patient, is disclosed. Generally, a first computer is utilized comprising a transceiver, software stored within the first computer defining functions to be performed, and a processor configured by the software to perform the steps of: developing a medical template, wherein the medical template comprises at least one medical protocol that is focused toward addressing a specific attribute of a medical ailment; and customizing the medical template to develop the patient care plan, wherein the patient care plan is focused on addressing the medical ailment to improve the health of the patient. The patient care plan is periodically updated after receiving updated medical information regarding the patient.

Abstract:

A method, system, and computer system for qualifying and selecting patients that are to be included in the electronic house call program are disclosed. A method to qualify and select patients to be included in the electronic house call program includes receiving an input that is associated with a patient. The input is assessed using at least one qualifying module, where the qualifying module has at least one predetermined parameter. Thereafter, a determination is made to determine if the input is a qualified input. Then the patient associated with a qualified input is enrolled into the electronic house call program.
