An applicator for applying a barbed fastener to a body tissue-is provided. The applicator comprises an elongated tubular portion having a longitudinal axis extending from a proximal end to a distal end and having a tube interior sized and configured for receiving the barbed fastener. An elongate driver is disposed at least partially inside the tube interior, the driver having a driver engaging portion at its distal end. The driver engaging portion is adapted to engage the barbed fastener in a firing position adjacent the distal end of the elongated tubular portion. The applicator also comprises; a driver actuator assembly adjacent the driver at its proximal end, the driver actuator assembly being adapted for selectively imparting an impulse force to the driver. The driver is configured for transmitting the impulse force to the barbed fastener.

A fastener is provided for inserting into a body tissue. The fastener has a head and an anchoring portion comprising a plurality of distally extending legs. Each leg terminates in an anchoring element configured to penetrate tissue and to cause a net lateral resistance force on the leg during penetration. The legs are configured and structured so that when the net lateral resistance force on the anchoring element exceeds a predetermined level, at least a portion of the leg is deflected laterally away from a pre-insertion configuration and so that when the net lateral force is subsequently reduced below the predetermined level, the at least a portion of the leg is biased back toward the pre-insertion configuration.

The disclosure also provides a driver apparatus for inserting the fasteners. The applicator device includes an indexer for sequentially indexing one or more fasteners, and a driver for applying an insertion force to a fastener that is sufficient to insert the fastener and attach it to body tissue. The disclosure also provides a method of using the fastener and applicator device.

Abstract:
A therapeutic device for correcting presbyopia in an eyeball having a ciliary muscle and a plurality of suspensory ligaments connecting the ciliary muscle to a lens includes an over-mold that mirrors a contact lens and fits against the eyeball. The over-mold encapsulates a micro thermoelectric generator (“μTEG”) that powers an electrical disc sub-assembly configured for sensing an activation of the ciliary muscle from a relaxed state, in which the ligaments are pulled taut and the lens is stretched into a flat position for distant focus, toward a contracted state, in which the ligaments become less taut and the lens moves into a rounded position for nearby focus. When the ciliary muscle is contracted, the sub-assembly stimulates the ciliary muscle with a minute electric muscle stimulation (“EMS”) to further contract the ciliary muscle, thereby further relaxing the ligaments to further improve nearby focus. Other embodiments are disclosed.
