Abstract:The implant guiding programmer is configured to automatically determine the defibrillation threshold (DFT) for a particular patient and to further determine an optimal implantation configuration for an implantable cardioverter fibrillator (ICD). In one implementation, to determine the DFT for the patient, the programmer correlates patient specific data with a predictive data base containing DFT information for an entire population of patients. The patient specific data includes the age, gender, heart size, chest size and medical history of the patient. The predictive database provides information correlating patient DFT with various ranges of the patient specific data for an entire population of data. In this manner, the programmer quickly approximates the DFT for the particular patient based upon the age, gender, heart size and medical history of the patient. In another implementation, the programmer automatically determines the DFT for the patient by performing a finite element analysis on an image of the thorax of the patient generated using an MRI or a CAT scan.

Abstract:Therapeutic shocks generated by implantable cardiac stimulators, such as implantable cardioverter-defibrillator devices, may result in residual charges being deposited on the defibrillation electrodes and/or polarization of the tissues surrounding the defibrillation electrodes. These parasitic charges and polarization may mask a possible ventricular fibrillation of the heart following the application of the therapeutic shocks because such a ventricular fibrillation is characterized by intrinsic signals of very small amplitudes. A small duration pulse is applied to the electrodes right after a therapeutic shock. In addition, or alternatively, the electrodes can also be shorted together to dissipate the parasitic charges and the polarization of the tissue. As a result, a ventricular fibrillation can be detected earlier than in previous cardiac stimulators.

Abstract:In an implantable cardioversion device, the condition of the patients heart is determined from an intrinsic ventricular parameter, such as the ventricular rate, and therapy is provided with a shock or pulse generator, including, for instance, antitachycardia pacing therapy or defibrillation shocks. Initially, the conditions, i. e. , ventricular tachycardia or fibrillation/flutter, are determined using predetermined values for a set of thresholds defining the various conditions. A generator applies the quantity of electrical energy to the heart responsive to the arrhythmia detector detecting the arrhythmia, when neither chamber is in its vulnerable period.
