    Methods and apparatus for performing arthoplasty utilizes a plurality of apertures created in a bone, each aperture having a cross section defined perpendicular to an axis of the aperture that intersects a plane of a resected surface to be created in the bone and also intersects a peripheral rim border that externally delineates the resected surface. A pin feature is inserted into each of the plurality of apertures and the resected surface is created by guiding a cutting tool along at least a line of contact of the pin features. An implant can then be attached to the resected surface.

    A cutting tool for creating resected surfaces of bone during arthroplasty procedures has a single offset handle arm that presents a narrower effective width than a width of the cutting profile of the cutting tool along a longitudinal axis of the cutting profile. Preferably, the single offset handle arm creates a cutting tool with a generally L-shaped outline simultaneously accounting for soft tissue/incision geometry and bony geometry to accommodate both in facilitating ease of use and minimal displacement of soft tissue during cutting. The cutting tool can be snaked into position through an incision approximately the size of the width of the handle arm, instead of requiring an incision that is essentially the width of the cutting path of the cutting tool. In alternate embodiments, the cutting tool is a milling tool, a wireplasty cutting tool, a band saw or reciprocating cutting tool.

    A cutting tool is provided with an arcuate cutting blade that preferably engages a guide tool to create a curved resected surface during an arthorplasty procedure. In one embodiment, a depth of the cutting blade is sufficient to permit the simultaneous creation of resected surfaces on two bones that articulate, such as both the femor and the tibia for a given condyle, without the need to reposition the guide or the leg. In another embodiment, a cutting member has a generally rectangular cross-section along a longitudinal axis with a first and second surface having cutting teeth defined thereon and a third and fourth surface adapted to interface with a cutting guide positioned proximate the bone. In this embodiment, the cutting tool can resect the bone in two different directions without reorienting the cutting member.

  • Abstract:
    Milling and drilling tools and soft tissue management techniques are provided for arthroplasty that facilitate intraoperative and postoperative efficacy and ease of use. In one embodiment, resiliently biased soft tissue protective sleeves surround a side cutting tool and are interposed along the longitudinal axis of the cutting tool, preferably adjacent each side of the bone. The soft tissue protective sleeves are biased to track along the contour of the side of the bone and prevent the cutting tool from being exposed to soft tissue during the resection. In another embodiment, a pilot drill is initially utilized to create an initial bore in the bone to be resected. The pilot drill preferably has an end cutting arrangement and a non-cutting removal channel that minimizes the tendency of the pilot drill bit to drift off-axis as the initial bore in the bone is created. Once the initial bore is created, the pilot drill bit is withdrawn and a second side cutting tool, such as a milling bit, is then inserted into the bore to perform the desired surface resection.

    A prosthetic implant utilizes lateral retaining structures as part of the interior surface of the implant to more effectively secure and retain the implant while reducing the overall size and mass of the implant. In one embodiment, the prosthetic implant is provided with one or more T-shaped members extending from the inner surface of the implant, with the cross-member of the T-shaped member forming the laterally retaining structure that mate with a correspondingly shaped channel formed in the bone and are inserted into that channel at one or more oversize locations along the channel. In another embodiment, the prosthetic implant is provided with one or more retentions apertures in a projection structure extending inwardly from the inner surface of the implant that are laterally secured with a force fitted cross pin inserted through the retention aperture.

In accordance with one embodiment, a guide structure is provided with one or more guide pivot aperture(s) and one or more guide pivot reference surface(s) that mate with a bushing assembly controlling a cutting tool. The bushing assembly possesses a bushing reference plan which mates with the pivot reference surface(s) of the guide structure and a bushing pivot pin which mates with the guide pivot aperture(s) of the guide structure. In one embodiment, a bushing guide lumen is operably coupled to the guide structure for articulated and/or axial guidance of the cutting tool.
